Over the past year several significant clinical studies were completed. One of the most important, published in December, reported that treatment with rivastigmine Exelon ; improved cognitive abilities of PD patients also affected with dementia. This 24-week study involved more than 400 subjects treated with a drug already in use for Alzheimer's disease. Rjvastigmine 3-12 mg ; was given daily and led to encouraging results. A moderate degree of clinically meaningful improvements occurred in some patients and overall, ratings were better than placebo. This treatment did not produce dramatic results, however, and some patients experienced nausea or other side effects. Rkvastigmine is one of five drugs currently approved for the treatment of Alzheimer's disease. This study provided the first large-scale assessment of the potentially important role that drugs of this class might offer for cognitive decline sometimes encountered in advanced PD. Another clinical study in 2004 generated a great deal of controversy and continues to do so. A few months ago, Amgen Corporation completed a study of an experimental treatment for PD called glial-derived neurotrophic factor GDNF ; and concluded that it was ineffective. GDNF was infused directly into the brain by a surgically implanted device in the hopes of restoring function in severe PD. The drug duplicates a naturally occurring protein found in the brain and effective at improving animal models of PD. When this technique was carried out initially in five patients, there were exciting results: all achieved marked improved without toxicity. Subsequently, a six-month study was carried out with placebo controls and a double-blind format for PD ratings. In this study, conducted in 34 subjects, GDNF offered no improvement and the placebo group actually did better. Furthermore, two potentially serious safety issues were encountered. An imaging technique did suggest that some degree of recovery occurred from GDNF treatment, although this was not detectable in clinical ratings. As a result, Amgen halted the study and cancelled further plans for GDNF development. Criticism to Amgen's decision developed swiftly from several PD patient groups. The key argument has been that study sponsor's decision was faulty and premature in ending the search for a potentially effective treatment. Several advocacy groups have demanded that Amgen make the drug available on a compassionate use basis and to conduct further clinical research. For its part, Amgen has stood by its scientific analysis of the data despite the initial 5-patient open-label experience ; , and has stated that further risk to patients from GDNF is not warranted. As one of the investigative sites planning to investigate GDNF, I was naturally disappointed in the GDNF results but glad that the protective and scientific perspective of modern clinical research keeps us on course in the search for safe and effective treatments and terbinafine.
