From neurons impaled with electrodes filled with 200 mM BAPTA, a Ca2 * chelator Tsien, 1980 ; . To confirm that BAPTA had been injected, we monitored the amplitude of the slow afterhyperpolarization sAHP ; , which is mediated by a Ca2 * -dependent K * current in hippocampal Lancaster and Adams, 1986; Lancaster and Nicoll, 1987; Storm, 1987 ; and neocortical neurons Schwindt et aL, 1988 ; . Figure 4 left ; illustrates the sAHP that is produced after a train of action potentials. This neuron was depolarized to a potential of -70 mV to enhance the sAHP. The amplitude of the sAHP was decreased after 20 min of impalement with the BAPTA-containing electrode Fig. 4 center ; . This can be seen most clearly in Figure 4-4 right ; where the two records are superimposed. The decrease in the sAHP typically required 20-30 min, at which point we applied TEA. PSPs evoked under these conditions Fig. 42?, left ; appeared similar to those recorded with electrodes not containing BAPTA. EPSPs evoked by weak stimulation were not enhanced in the presence of TEA Fig. 4B, center, 105.1 19-1% of control; P 0.05 ; and were depressed slightly after return to control ACSF for 20 min Fig. 4B, right; 86.4 11.3% of control; P 0.05; n 4 ; . This is seen most clearly in the inset where the EPSP evoked prior to TEA application is superimposed on the EPSP evoked after 20 min of control ACSF. In contrast, the amplitude of the presumptive IPSP evoked by strong stimulation was 1835 40.6% and 193.4 43.3% of control both P 0.05 ; when measured at the same time points. The blockade of TEA-induced enhancement of the amplitude of the EPSPs evoked by the test stimulation intensity is shown clearly in Figure AC, which summarizes four experiments. Pharmacological antagonism of L-type VDCCs prevents the induction of LTPK in the hippocampus Aniksztejn and Ben-Ari, 1991; Huang and Malenka, 1993; but see Hanse and Gustaffson, 1994 ; . Ca2 * entry into the postsynaptic neuron via this class of VDCC is believed to be critical for the induction of LTPK- TO test, for instance, metabolism.

In addition to the interventions previously noted, consider the following actions while awaiting EMS: Remove any constricting clothing or jewelry the student is wearing; label it and secure in a safe place. Apply sterile dressings to all open soft tissue injuries. Elevate suspected fractures, splint or stabilize them, and apply cold packs. Stabilize any impaled object--do not remove it. Report immunization status and other pertinent health history to EMS and hospital personnel. Provide emotional support and reassurance.

Control who stopped warfarin is uncertain, and additional research is needed to address this important question. Depending upon symptoms, rate control may be reasonable initial therapy in older patients with persistent AF who have hypertension or heart disease. For younger individuals, especially those with paroxysmal lone AF, rhythm control may be a better initial approach. Often medications that exert both antiarrhythmic and rate-controlling effects are required. Catheter ablation should be considered to maintain sinus rhythm in selected patients who failed to respond to antiarrhythmic drug therapy.340 8.1.2.3. Effect on symptoms and quality of life. Information about the effects of antiarrhythmic and chronotropic therapies on quality of life is inconsistent.292, 294, 295 The AFFIRM, 293, 296 RACE, 293, 295 PIAF Pharmacologic Intervention in Atrial Fibrillation ; , 342 and STAF Strategies of Treatment of Atrial Fibrillation ; 343 studies found no differences in quality of life with rhythm control compared with rate control. Rhythm control in the PIAF and How to Treat Chronic Atrial Fibrillation HOT CAFE ; 344 studies resulted in better exercise tolerance than rate control, but this did not translate into improved quality of life. In the Canadian Trial of Atrial Fibrillation CTAF ; study, 347 there was no difference between amiodarone and sotalol or propafenone as assessed by responses to the Short Form-36 questionnaire, while a symptom severity scale showed benefit of amiodarone over the other drugs. In the Sotalol Amiodarone Atrial Fibrillation Efficacy Trial SAFE-T ; , 292 restoration and maintenance of sinus rhythm in patients with AF significantly improved quality of life in certain domains, but amiodarone was associated with a decrease in mental health function compared with sotalol or placebo.292 Symptomatic improvement has also been reported after the maze procedure in patients with AF.348 In a substudy of AFFIRM, there was no significant association between achieved HR and quality-of-life measurements, New York Heart Association functional class, or 6-min walking distance in patients with AF compared with less well-controlled patients.345 On the whole, rate- and rhythm-control strategies do not affect quality of life significantly or differently. Even when sinus rhythm can be maintained, symptoms of associated cardiovascular conditions may obscure changes in quality of life related to AF. Clinicians must exercise judgment, however, in translating shifts in quality of life in these study populations to the sense of well-being experienced by individual patients. Patients with similar health status may experience entirely different qualitiy of life, and treatment must be tailored to each individual, depending on the nature, intensity, and frequency of symptoms, patient preferences, comorbid conditions, and the ongoing response to treatment. Long-term oral anticoagulant therapy with vitamin K antagonists involves multiple drug interactions and frequent blood testing, which influences quality of life in patients with AF. Gage et al. 349 quantified this as a mean 1.3% decrease in utility, a measure of quality of life in quantitative decision analysis. Some patients 16% ; thought that their quality of life would be greater with aspirin than with oral anticoagulants, despite its lesser efficacy. Other investigators, using decision analysis to assess patient preferences, found that 61% of 97 patients preferred anticoagulation to no treatment, a smaller proportion than that for which.

Disturbances, vasomotor symptoms, mood changes, cognitive impairment, and sexual dysfunction comprise the most commonly reported menopause-related symptoms.23 Treatment strategies, therefore, may address these symptoms individually or in different combinations. Numerous studies have evaluated the benefits and risks of hormone therapies either as estrogen therapy or as combination estrogen-progestin ; for mood symptoms and depression during the perimenopausal and postmenopausal years. Positive findings have been demonstrated with estrogen therapy for depression during perimenopause 36-39 but not for postmenopausal depression.36, 40, 41 Evaluations of hormone therapy for sleep disturbances during menopause have also shown mixed results. Estrogen has been reported to improve sleep in women with hot flashes by reducing the number of awakenings and total nighttime wakefulness and improving sleep efficiency.42-44 However, some studies have failed to demonstrate a significant impact of estrogen therapy on sleep characteristics.45 The use of sedative-hypnotic agents for sleep disturbances during menopause has also been explored table 3 ; . The ultimate goal of the therapy for managing sleep disturbances in women during menopause remains the same as that in other patients, specifically, to improve sleep while minimizing the risk of next-day sedation and impaired function. Benzodiazepines, although effective hypnotic agents, have been associated with next-day sedation and decline in cognitive.
