The role of Pharmacy Assistance Programs Many states have recently tried to extend access to medications while controlling pharmaceutical costs and encouraging rational, cost-effective prescribing. These efforts, usually called Pharmacy Assistance Programs, vary in the populations they cover and in the ways they define benefits. [American Association of Retail Pharmacists, 2002] For example, the Oregon Health Resources Commission subcontracts with the Oregon Health and Sciences Evidence-Based Practice Center for assistance in making transparent, evidence-based evaluations of specific classes of pharmaceuticals for inclusion on the state health plan list. [Office for Oregon Health Policy and Research, 2002] Michigan has instituted a reference-based pricing system similar to that in British Columbia. [Michigan Department of Community Health, 2002] The role of the Department of Veterans Affairs The Department of Veterans Affairs is the largest single purchaser of pharmaceuticals in the United States. It has instituted an evidence-based national formulary system in order to standardize its benefits package and contain costs. In addition to establishing a "partially-closed" formulary with restrictions on some drug classes ; , the formulary system includes generic and therapeutic drug substitution to minimize costs. The role of the private sector In the private sector, most drug benefits plans are now administered by pharmacy benefit management firms PBMs ; . PBMs serve as intermediaries between pharmaceutical companies, physicians, pharmacies and third-party payers. [ACP, 2001] PBMs develop formularies based on the recommendations of pharmaceutical and therapeutics P and T ; committees and cost considerations. They negotiate discount prices and dispensing fees with retail pharmacies to buy pharmaceuticals at bulk discounts. They also receive rebates from manufacturers, a percentage of which they share with the health plan or employer that contracts with them. PBMs use various strategies to contain costs and influence prescribing patterns. These include closed or partially restrictive formularies, limitations on the number of prescriptions and refills and limitations on the size of prescriptions. [Academy of Managed Care Pharmacy, 2000] They also use various methods of cost sharing with patients, such as tiered payment systems. [Fox PD et al., 1999] With the permission of physicians, they will substitute generic or formulary drugs in the same class for prescriptions that are made for off-formulary products. [Eber B et al., 2001] To influence prescription practice, they do prospective and retrospective drug utilization reviews of physician prescribing. PBMs sometimes set up disease management programs for people with potentially high-cost conditions, such as diabetes and heart disease. These programs encourage medication compliance and help patients take a more active role in managing their condition. [Eber B et al., 2001].

Adolf W. Karchmer, MD, Beth Israel Deaconess Medical Center, Harvard Medical School Over the past 20 years at Beth Israel Deaconess Medical Center, we have seen fewer amputations associated with diabetic foot infections and increased frequency of reconstructive vascular surgery, largely due to our use of a multidisciplinary approach--relying on the expertise of vascular surgeons, podiatrists, infectious disease physicians, and diabetologists-- to address infections and utilization of appropriate antibiotic therapy. Newer antibiotic agents present valuable treatment options for the medical management of diabetic foot infections. However, during consideration of resistance patterns, certain limitations of antibiotic therapies become apparent. Ertapenem has anaerobic, Gram-negative with the exception of Pseudomonas aeruginosa ; , and Gram-positive coverage, including activity against staphylococci and streptococci, making it an ideal choice for the treatment of diabetic foot infections, in which multibacterial infections are fairly common. However, ertapenem does not cover methicillin-resistant Staphylococcus aureus MRSA ; , which has become a more prominent concern over the past several years. Therefore, although ertapenem has a broad spectrum of activity and good tolerability, it has a limitation in that it does not cover MRSA. Linezolid, on the other hand, is an agent that covers MRSA, in addition to having good activity against Gram-positive organisms, and has the added benefit of availability in an oral formulation. However, linezolid also has limitations in that it does not have activity against any of the Gram-negative rods nor does it provide good coverage of Gram-negative anaerobes. Tigecycline has a slightly better spectrum of activity--covering anaerobes, Gram-negatives, and MRSA--however, it can only be given intravenously and it may cause nausea, raising concerns regarding its tolerability in certain patients. Additionally, as it is currently being investigated for the treatment of diabetic foot infections, there is limited clinical experience with tigecycline and unknown variables exist with its use. Although the broad variety of available antibiotic treatments affords clinicians many options in selecting an appropriate agent for the management of diabetic foot infections, drug selection must be considered carefully and individualized to each patient. Effective May 1, 2003 the B.C. government has established a new income based pharmacare program combining two existing plans the universal plan and the seniors' plan. The new program is called Fair PharmaCare and the changes are expected to increase group plan costs for employers with employees in B.C. by 8 to 12% depending upon the carrier and the drug benefit provided by the employer. Residents of B.C. must register for the program to be eligible. Registration for the Fair PharmaCare program can be accommodated on line at : pharmacare.moh.hnet.bc . Employers should be pro-active in communicating this change to their employees and encouraging registration to lessen the impact of this change on plan costs. The insurance carriers are expected to communicate their position with respect to the impact of this change shortly. The major changes to the drug program for individuals under 65 years of age are: The current flat $1, 000 deductible will be replaced with an annual deductible ranging from 0% to 3% of net family income with the percentage being dependant on the income reported. The $2, 000 out of pocket maximum for drug expenses is being changed to an family income based maximum ranging from 2% to 4% of family net income and pravachol. 1. Show Group Table 2. Show Summary 3. Dear has performed an independent review of the medical records of the abovenamed case to determine medical necessity. In performing this review, reviewed relevant medical records, any documents provided by the parties referenced above, and any documentation and written information submitted in support of the dispute. The independent review was performed by a matched peer with the treating health care provider. This case was reviewed by a physician who is Board Certified in Neurosurgery. Clinical History: This 42-year-old woman was injured on the job on . She was treated conservatively for a period of time for neck, arm, low back and leg pain. In October 2001, she underwent an anterior cervical discectomy and fusion at C4-C5 and C5-C6 with anterior plating for cervical spondylosis with spinal cord compression. Reports reveal she has severe degenerative disc disease at L3-L4, L4-L5 and L5-S1 and continues to complain of pain in the same areas mentioned above. Because of recurrent CMV pneumonia, she is not thought to be a surgical candidate for the required extensive lumbar spinal surgery. Throughout this period of time she was given large doses of Xanax, Soma and Valium simultaneously. Disputed Services: Pepcid, alprazolam Xanax ; , soma, promethazine Penergan ; , diazepam Valium ; , hydrocodone APAP, Protonix and BioFreeze during the period of 08 through 05 08 02. Decision: The reviewer partially agrees with the determination of the insurance carrier. The reviewer is of the opinion that Protonix, Pepcid and BioFreeze were medically necessary in this case. However, Xanax, Soma, Phehergan and hydrocodone APAP were not medically necessary in this case. So, how many of you have seen your mother hoist an antique dining room chair over her head and, shrieking, send it crashing down the basement stairs? Uh, me neither. My mother is the epitome of the wellmannered lady. She is still recognizably the gentle first grade teacher who was loved by classrooms of kids in the early 1940s. She would no more toss a chair down the stairs than she would swear at the Pope. But my kids saw me toss that chair. And they've heard me holler when I run into the corner of a table, and yell when I misjudge the distance between my face and the cabinet door. They've also heard me blurt out words that I never heard coming from my mother's mouth. Ah, you're saying to yourself. Impulse control problems. This woman needs medication. Ah, I say right back. I tried medication. Medication had me throwing the stinkin' chair. I didn't want to take drugs. In fact, I didn't want to admit to an ADD diagnosis at all. At the ripe old age of somewhere north of 40, I was neither ready nor willing to discuss a personal problem with ADD. I thought I was just overwhelmed, undernourished and underslept in the way that any mother of young kids would recognize. What I can see now, on looking back, is that my brain was challenged in a completely different manner than just having kids could explain. At the time, I couldn't see that the temper tantrums mine ; , lost keys ditto ; , and inability to think about planning meals, much less actually shopping for and cooking them in a timely manner, meant anything other than that it was my kids' fault: after my second child arrived, my responsible adult brain went on permanent hiatus. I remember complaining to a friend about how I felt. "You just feel that way because you have young kids, " she said, looking at me as were a whining two-year-old. "We all feel like that." And yet, and yet.her house was uncluttered, her meals were on time and nutritious, she didn't lock herself out of her car. Conclusion: Some of us are competent. Some of us are not. It was obvious where I fell. And yet I hadn't felt incompetent before. What changed? Four years later, I'm still not sure what changed. Maybe having multiple kids brings out latent ADD-related behavior. I ended up seeing a therapist who sent me reeling with her suggestion that I might be experiencing something more than just Mommy Overwhelm. When a psychiatrist confirmed the therapist's idea and suggested medication, I resisted. I denied the diagnosis despite all evidence to the contrary "Oh, my gosh, this sounds just like you, " my husband said when I showed him a "You might have ADD if you.
