TUESDAY, Jan. 24 HealthDay News ; -- They may be great for the heart, but the omega-3 fatty acids found in fish and fish oil supplements do nothing to prevent cancer, a major analysis finds. A new review of more than 38 studies on the subject finds no evidence that diets rich in fish fight any kind of malignancy. Omega-3 fatty acids "definitely have health benefits, but they are not a panacea. Preventing cancer is not one of the things omega-3 fatty acids do, " said lead researcher Dr. Catherine MacLean, a natural scientist at Rand Health and a rheumatologist at the Greater Los Angeles VA Healthcare System. The study, supported by the U.S. Agency for Healthcare Research and Quality AHRQ ; and the National Institutes of Health, appears in the Jan. 25 issue of the Journal of the American Medical Association . An earlier meta-analysis, also funded by AHRQ, found that diets rich in omega-3 fatty acids did have a beneficial effect on cardiovascular health. But evidence of any protection against cancer has been more elusive. "There was a plausible mechanism, " MacLean said. "Omega-3s are integral to some of the inflammatory pathways that are also common to cancer, so the idea was that if you had more omega-3s maybe that would dampen this inflammatory process." But proof for the theory was scanty. "I think there were a lot of high hopes, but very little evidence, " said AHRQ director Dr. Carolyn Clancy. While some research -- mostly animal studies -suggested omega-3s might have an anti-cancer effect, other studies found no such link. "So the office at the NIH that deals with dietary supplements asked AHRQ to do a very rigorous review of the studies that had been done, " Clancy explained. In the study, MacLean's group analyzed data from studies conducted over the past 40 years. The vast majority showed no effect of even high-dose omega-3 fatty acids on the incidence of a wide range of cancers, including breast, colon, lung and prostate malignancies, the researchers found. Colleen Doyle, director of nutrition and physical activity at the American Cancer Society ACS ; , said the finding was "not surprising, " since the evidence had always been slim that the nutrient might fight cancer. But she added that the results would not change current ACS dietary guidelines. "We will still recommend that people include fish in their diets. Target hardening devices include metal detectors, surveillance cameras, motion detectors, identification-badge systems, and security gates to block off corridors. After Columbine, Lockwood Greene Technologies, a company specializing in corporate security, conducted a security analysis of Jefferson County schools and recommended the installation or the employment of many such devices. The firm also proposed several fundamental changes in practice designed to increase security at local schools. Some of the proposed practices are relatively inexpensive and would seem to make good sense at many schools. For example, Lockwood Greene Technologies proposes that schools issue identification badges for visitors, which make it easier for faculty and staff to ascertain whether strangers in a building have permission to visit. This, too, would appear to be a sensible plan for many schools troubled by the frequent presence of unauthorized persons within their buildings. Nevertheless, it will not forestall typical cases of school violence because their perpetrators are usually members of the student body, not outsiders. Lockwood Greene Technologies proffered certain suggestions addressing more directly the Columbine High School situation, in that they recommend the imposition of restrictive controls on items that students are allowed to bring into a school building, and the number of entrances into a building. For example, one proposal would limit main school entrances to two, with school personnel assigned to monitor them at the start and end of each school day, as well as during passing periods and lunch periods. Other school entrances would remain locked against outside entries, and alarms would sound if they were opened from inside the school building. Backpacks would be randomly searched in an effort to deter students from bringing weapons and dangerous devices into the school although it should be noted that Fourth Amendment problems are presented if such a policy is adopted by a school administration directed at students concerning whom there is no reasonable suspicion at the time of search; cf. the Supreme Court's seeming rejection of random drug-usage testing of students not involved in competitive sports activities, Vernonia School District v. Acton, 515 U.S. 646 1995 ; , and of pregnant patients at public hospitals, the results to be transmitted to prosecutors, Ferguson v. City of Charleston, 121 S. Ct. 1281 2001. As you are aware all Corporations are required to submit their Annual Return of Information not the income tax return ; as required under the Corporations Act, Manitoba, along with the pharmacy licence renewal each year. Some pharmacies this past licensing period have mentioned they would have to get a copy from their lawyers for a fee ; . At the time the Annual Return of Information is submitted, the pharmacy owner should request a copy to be kept on file until the pharmacy licence renewal is sent in. Poor HbA1c control indicates that either an HbA1c level was greater than 9.0 mg dL, or that no HbA1c test was conducted during the measurement year. This is an important distinction since members who did not have an HbA1c test during 2005 may not necessarily be in poor control. Table 42 below displays the results of poor HbA1c control based on documented HbA1c levels, for instance, nevirapinne pharmacokinetics. Were sufficient for a sponsor to come in with an application for a drug without going through the clinical trials-the normal process of giving the drug to patients and watching for the clinical effectiveness. The third element of our policy was an articulation of the bioequivalence in the composition that would be needed for approval. This is what the FDA does on a day-in and day-out basis--it looks at bioequivalence and stability of drugs. But here, again, we were proactively articulating what the threshold would be for the approval of these combination products. The fourth and final element was a pledge on time framesthat the FDA would review these products in time frames as short as three to six weeks. Shortly after we announced these guidelines, companies expressed interest in developing new combination products. In particular, two groups of branded companies issued press releases expressing interest in developing fixed-dose-combination drugs. We're hopeful that other companies will step forward as well. Despite the innovative steps taken by the FDA, an enormous political debate surrounded the announcement of our new policy. As many of you know, there was a lot of talk about the Indian companies that were developing fixed-dose-combination drugs--principally, Cipla's product, which consisted of Sustiva, Nevirapine, and Lamivudine. The discussion focused on whether the U.S. would use the president's fund to procure these products. The reason that people in the AIDS community, particularly in the international AIDS community, wanted us to use our funds to procure these products is because they were cheap fixed-dose combinations. The fact that they were fixed-dose combinations is not debatable, and the fact that fixed-dose-combination drugs will probably offer some benefit in a Third World environment without a lot of health-care infrastructure probably isn't debatable, either. The assertion that these drugs were cheaper than other available options is probably very debatable, but I won't get into that today, since it is not my direct area of expertise. What I would like the audience here today to think about. Published guidelines recommend the use of antiretroviral therapy for patients infected with HIV.14 Widely used antiretroviral drugs available in the United States include the protease inhibitors saquinavir, indinavir, ritonavir, and nelfinavir ; and the nonnucleoside reverse transcriptase SLIDE 72 inhibitors NNRTIs ; nevirapine, delavirdine, and efavirenz ; . However, these inhibitors interact with rifamycin derivatives, such as rifampin and rifabutin, which are used to treat and prevent the mycobacterial infections commonly observed in HIV-positive patients. Of the rifamycins, rifampin has the most potent interactions; rifabutin has substantially fewer interactions. Because the most recent recommendations for the use of antiretroviral therapy strongly advise against interruptions of therapy, 16 and because non rifampin-containing alternatives for TB treatment are available, previous antituberculosis therapy options that involved stopping antiretroviral therapy to allow the use of rifampin are no longer recommended. The other class of antiretroviral agents available in the U.S., nucleoside reverse transcriptase inhibitors NRTIs ; , which include zidovudine, didanosine, zalcitabine, stavudine, lamivudine, and abacavir, are not contraindicated with the use of rifamycins and do not require dose adjustments.
