For some people in the early and middle stages of alzheimer's disease, the drugs tacrine cognex ; , donepezil aricept ; , rivastigmine exelon ; , and galantamine reminyl ; are prescribed to possibly delay the worsening of some of the disease's symptoms. 9. 4 lungs, the gastrointestinal tract, and organs innervated by the cranial nerves. Nicotinic receptors affect skeletal muscles and the sympathetic nervous system Grue et al. 1997 ; . Importantly for the purposes of the present discussion, muscarinic ACh receptors have also been identified as central synaptic elements within neural pathways important for learning, memory, and behavior in vertebrates Williams and Messer 2004 ; . Nicotinic ACh receptors have also been associated with impacting memory Grn et al. 2006 ; . Grn et al. 2006 ; examined the introduction of galantamine, which is an acetylcholinesterase inhibitor and modulator of nicotinic ACh receptors in patients suffering from Alzheimer's disease. From this study they concluded that the introduction of galantamine leads to an improvement in episodic memory and increased performance in a spatial navigation task. The inhibitor had no effect on attention, short-term or working memory, and no specific improvement with overall hippocampal function Grn et al. 2006 ; . Experiments examining the introduction of neurotoxins which form lesions in forebrain cholinergic neurons have helped to strengthen the correlation between ACh and learning as well as memory Pepeu and Giovannini 2004 ; . Increases in extracellular ACh concentrations of forebrain cholinergic neurons have been demonstrated in tasks in which an animal must analyze a new stimulus which may offer a threat or a reward. Orsetti et al. found that hippocampal ACh output has been shown to be higher during acquisition of a rewarded behavior, but no increase in ACh output was observed during its recall. Acquas et al. concluded in 1996 that cortical ACh release was more prominent during visual and auditory stimuli, but when either stimulus was reintroduced, ACh output was not as significant due to habituation Pepeu and Giovannini 2004 ; . Therefore, the activation of cholinergic synapses can be correlated in behavioral activities like acquisition and interaction with a novel stimulus.

100 Years later. Earlier diagnosis, new drug therapies and expanding research show that the story is changing in the fight against Alzheimer's disease. There are currently four medications which have been approved for use in Canada in treating Alzheimer's disease: Aricept donepezil ; , Exelon rivastigmine ; , Reminyl galantamine ; , and Ebixa memantine ; . Aricept, Exelon and Reminyl are useful in treating mild to moderate Alzheimer's disease while Ebixa is useful in treating moderate to severe dementia. Potential benefits from these drugs include improved cognition, behaviour and function. Two new drugs which are undergoing clinical investigation are Flurizan and Alzhemed. Flurizan, in clinical trials, seems to have the potential to not just treat the symptoms of Alzheimer's disease but also to slow down the progression of Alzheimer's disease in its early stages. It has an antiamyloid action which gradually reduces the build-up of the toxic protein in the brain. Over a one year period in both Canada and the U.K., Flurizan was tested in 207 patients with mild to moderate severity of Alzheimer's disease. The results were encouraging. A larger phase 3 clinical trial is now currently underway in the U.S. with hopes for a second Canadian and European trial in late spring of 2006. While interest in the drug appears warranted, it will likely be several years before the treatment is approved for use in Canada by Health Canada. Alzhemed is another drug that is designed to prevent amyloid formation and deposition in and glibenclamide.

Seroxat & Withdrawal - 3 Thus there appears to have been a failure by companies to seek and inform regulators of possible problems. This makes the reports of withdrawal following the marketing of SSRIs less surprising than might otherwise have been the case. From shortly after the licensing of this drug on the UK market, the Medicines Control Agency was flooded with reports of withdrawal problems for Seroxat. In 1993, the Drug Safety Research Unit study of Seroxat noted a high frequency of general practitioner reporting of withdrawal problems although the DSRU and glucovance, for example, acetylcholinesterase inhibitors.

As with all drug treatments, numerous regulatory issues can influence both its availability and use in society. These issues not only influence whether patients will have access to the drug, but will also have an impact on health practitioners diagnosing and treating Alzheimer's disease. The drug is exclusively marketed for the treatment of mild to moderate Alzheimer's disease 7, 11 ; . There has been no mention of potential drug abuse in the literature, suggesting that this drug does not have the capacity to be applied in such circumstances. 6.1 Marketing issues One of the major marketing issues of this year revolves around the renaming of galantamine. The trade name was originally Reminyl, but was recently renamed Razadyne due to confusion with the diabetes drug, Amaryl glimepiride ; . This had led to several dispensing and prescribing errors between the two drugs 28, 29 ; . To avoid further errors and improve patient safety, the decision was made to change the name to something less problematic. 6.2 Availability and funding issues Another important issue that has been thrust into the spotlight is the apparent funding and possible withdrawal of three drugs commonly used to treat Alzheimer's disease. This has created intense controversy in Britain, regarding the National Institute of Clinical Excellence NICE ; proposal to discontinue donepezil, rivastigmine and galantamine in patients suffering from mild to moderate Alzheimer's disease 30 ; . Although there is no direct evidence disputing their effectiveness and safety, it is their cost-effectiveness Advances in psychiatric treatment ; that has come under scrutiny. NICE argue that these drugs are not considered cost-effective to warrant a cost of 60 million pounds per year 31 ; . Of course, this has created a wave of anger from patient groups, because most of the drugs available for treating this disease are cholinesterase inhibitors 19 ; . One of their recommendations is that new patients will not be prescribed them, while those patients currently receiving them will continue to do so Without access to cholinesterase inhibitors, patients may not seek medical attention for memory difficulties and other symptoms of the disease. The overall outcome will be a healthcare system under constant pressure to delivery health services, thus increasing the cost of care 33 ; . Since diagnosis and treatment will no longer be a priority, this leaves few options for Alzheimer's sufferers and their caregivers and inderal. Table 4. Medicine pricing issues have always been of great concern for developed and developing countries due to the high cost incurred in pharmaceutical care. In developing countries the studies and data on medicine prices are scant. Despite the fact that several stakeholders recognize medicines prices are an issue; the issue could not be addressed in a systematic manner in the past due to lack of a reliable methodology. However, the World Health Organization and Health Action International have undertaken a commendable task to address methodological difficulties in surveying medicine prices. This report is an outcome of a systematic study employing the World Health Organization Health Action International WHO HAI ; methodology and is an attempt to address the pricing situation in Malaysia. We strongly believe that the findings and suggestions given herein would be helpful and useful for devising an effective pricing policy. The policy so formed can positively influence health expenditures. Lished last year was the first prospective study to evaluate the effects of a statin exclusively in patients who have undergone percutaneous coronary intervention PCI ; procedures such as angioplasty. These patients are at a high risk of a second major adverse cardiac event. The LIPS findings showed that Lescol 80 mg significantly reduced major adverse cardiac events by 22%, even in patients with normal cholesterol levels. Moreover benefits of Lescol were even more profound in high-risk patients with diabetes and multi-vessel disease. Novartis has filed regulatory applications for the additional indication of secondary prevention of cardiovascular events in patients who have undergone PCI procedures. If approved, Lescol could help the 1.8 million patients worldwide who undergo angioplasty procedures annually. Lamisil terbinafine ; , used in the treatment of fungal infections of the nails, extended its share of US prescriptions to more than 80% and achieved a 4% increase in a declining market. Elidel pimecrolimus ; was launched in 13 countries, including the US. Sales reached CHF 148 million, and within just six months this highly effective, non-steroid cream became the number-one branded treatment for eczema in the US. Exelon rivastigmine ; for the treatment of Alzheimer's disease posted strong sales growth of 26%, lifted by the European approval of a labeling expansion to include the product's unique property of inhibiting two enzymes believed to be related to the cause Alzheimer's disease. During 2002, European regulatory officials approved the additional labeling of Exelon as an inhibitor of both acetylcholinesterase AChE ; and butyrylcholinesterase BuChE ; . Recent research suggests that BuChE may play an increasingly important role in regulating brain acetylcholine levels as Alzheimer's disease progresses and that the dual inhibitory action of Exelon may provide additional treatment benefits for patients with the disorder. This makes Exelon unique in its class as the other cholinesterase inhibitors commonly used to treat Alzheimer's disease, such as donepezil and galantamine, inhibit only AChE. Trileptal oxcarbazepine ; , used in the treatment of epilepsy, continued to post substantial sales growth 91% ; , supported by increased use as a monotherapy and lexapro.

Language in the legislation and some aspects of concern around the accountability of the executive officer. I'm going to allow my colleague Mr. Viti to take us through those two areas. Mr. Frank Viti: Specific to interchangeability, the Get it Right for Patients Coalition was highly alarmed by the initial language in Bill 102 that would, we felt, expand the interchangeability to include similar drugs, not only same-to-same drugs. We also were concerned about the new dispensing powers that would be extended to the dispenser, or pharmacist, which we felt would allow automatic switching of drugs, even when they were not designated as interchangeable and were similar. So that has caused us a lot of concern over the last two weeks. Once again, we have concern with an issue specific to what accountability or re-review process exists when the executive officer has deemed a drug not to be listed on the formulary. With those two areas of concern, we engaged in a comprehensive discussion with the minister and members of his staff. We want to thank, first and foremost, the minister and every member of his team for finally listening to the patient voice. As patients, we have only come together as a coalition recently. We were welcomed, and they have been listening to our issues and concerns. Moving forward, we believe that the Get it Right for Patients Coalition and the minister, as of today, have an understanding--we don't have any documents, but we have an understanding--that the minister and his team are committed to protecting patients' health and that there will be some changes in terms of interchangeability. We are confident that the current DIDFA legislation will protect patients when drugs, on the rare occasion, are deemed similar, and that the no-substitution clause will end up protecting, on most occasions, patients' health. We have helped the minister think through a mechanism that will work to have a re-review of a negative decision specific to the executive officer not listing a drug. We will continue to work with his team specifically toward a process that we're confident will protect patient health. Finally, we want to outline four specific detailed recommendations for your committee to consider that we think would make Bill 102 a very strong piece of legislation, and which would not in any way compromise health outcomes. We'd like to take you through these four very detailed recommendations. First and foremost, an amendment, subsection 1.1 3.1 ; , which defines similar active ingredients for the purposes of interchangeability in subsection 1.1 3 ; : We would recommend strongly the adoption of these new definitions of interchangeability. Basically, the bottom line is that no interchangeability for same-to-same and same-to-similar drugs without the no-substitution protection will be protected. No substitution when chemical entities are not the same would also be protected. The second issue, the deletion of section 3: Louise, maybe you can take us through it. Jay jacobson will travel to san-francisco this april 15th-17th for the american college of physicians annual conference to participate in two workshops on medical ethics.

An independent review of quinine in the December issue of NPS RADAR alerts health professionals to the risks of quinine as a treatment for preventing muscle cramp. "No oral quinine products are now TGA approved for muscle cramp. Quinine for the treatment of cramp has been deleted from the Pharmaceutical Benefits Scheme PBS ; in December 2004. The potential severity of adverse effects associated with taking quinine far outweighs any potential benefits, " said Dr Peter Roush, Chair, NPS New Drugs Working Group and Brisbane GP. "NPS RADAR reports quinine only has limited benefits for muscle cramp, yet it can cause severe thrombocytopenia which is rare, unpredictable and potentially fatal. "GPs now need to consider and advise patients of alternative treatments such as passive stretching and massage of the affected muscle. Whilst there are no clinical trials to support these options, there is very low risk of adverse effects, " concluded Dr Peter Roush. Patients who have had quinine-induced thrombocytopenia should be advised to avoid all products containing quinine, including bitter lemon and tonic water. Quinine is still listed on the PBS to treat malaria as the benefits of quinine in treating this serious disease outweigh the risk of harm to these patients. The December issue of NPS RADAR also includes independent reviews of the prescription medicines: Galantamine Reminyl ; prolonged-release capsules for dementia in Alzheimer's disease; Sertraline Zoloft ; and fluoxetine Lovan, Prozac ; for premenstrual dysphoric disorder; and Orlistat Xenical ; for obesity, now available over-the-counter.
