Hospital would not confront the Donaldsons with the probability of death they could not bring themselves to face Elmira with it. Still Elmira's spirits did not flag. The previous week, a second nurse, Margaret Mitchell, 20, had been rostered to Turner Ward. She was like Jill, a born-again Christian, a person who had given control of her life to Jesus Christ. If I had asked with her or Jill to explain such a coincidence they would have replied it was no coincidence, it was God's will. She had read principals of Elisabeth Kubler-Ross author of On Death and Dying, as part of her training. She had strong views on the quality of life as well as the quantity and was please to discover that some of this was practised in the hospital. Jill too had read Kubler-Ross and she struck up an immediate rapport with Mitchell. The friendship proved a support to her and Des in the few weeks to come. But initially Mitchell had to combat the urchin Elmira who immediately pulled the I-canmake-you-say-black trick on her. Mitchell retaliated by fetching her younger sister's 101 elephant jokes book to fight fire with fire. Sample: How do you get down from an Elephant? You don't. You get down from a duck! As healthy as was Elmira's wit, it was evident she was losing weight, losing vital strength. September 7 brought a traumatic day for her, her parents and the hospital staff as they attempted to introduce a nasal gastric tube to feed her. Elmira was violently opposed to it. Jill stayed that night and the next morning Elmira vomited the tube out. They tried again but could not even get it her throat. After another week, in which Elmira only picked at her food, the hospital proposed intravenous feeding. Dr Francis knew from Elmira's records that it has been unsuccessful before, but he had a few options. Without it Elmira was becoming progressively weaker, It was an unhappy decision for him and the Donaldsons. That meant Elmira's sojourns at home ended. When she returned to the hospital that Monday evening she stayed. And never left again. Now, even on someone like Elmira, the strain began to tell. The next morning she showed how near the surface was her concern at the unknown quantity of death. At the end of one physiotherapy session she was so short of breath she began to gasp. She panicked and cried out: "I'm dying! I'm dying!" Jill calmed her. Later she asked Elmira how she knew she was dying. "Because I can't get enough oxygen in my lungs, " said Elmira, quoting from the medical books she had been read. Two days later, Elmira's bed was moved to be placed in front of the Turner Wards main desk to be under consent supervision. She began to wear her oxygen mask full time, instead of just at night. From experience this signalled to Jill that Elmira had only a week to live. It was September 18. Next day, the fourth day of intravenous feeding, unit director Dr Francis asked the Donaldsons for permission to stop it. Fluids introduced directly into the blood stream increased the volume of fluid Elmira's already overstrained and failing heart had to pump, It was causing distress, said Francis. It had been a risk in which the calculations were that if Elmira did not benefit it should not produce any irreversible condition. The halt, in fact, signalled the hospital's acceptance that they were losing Elmira. It was no small admission. Turner Ward had known Elmira all her life it seemed, a life which they were instrumental in extending. The moment when the staff came to the belief was not perceptible in their manner, because Elmira was not the only child in their ward, all of whom depended as.

Been self-employed [as a plumber] in the past, and realised what it would cost the company to allow me to spend time in Banda Aceh." Matt eventually spent six weeks in the devastated city. "I assumed plumbing would occupy most of my time, " Matt said. "I'd been briefed prior to leaving for Banda Aceh and was nervous about whether I could fill my role provide sanitation and a clean supply of water to the orphanage." Initially, Matt involved himself solely in improving the rudimentary plumbing systems hurriedly installed by the doctors and medical staff. His responsibilities also extended to maintenance and construction. He built stools, storage space and doors to protect the simple buildings housing the orphanage's inadequate supplies. Matt also dug his share of trenches, unloaded a never-ending line of supply trucks and so on. "But I also wanted to do my bit with the kids, " Matt said. "I even got involved in English teaching. The word got around that we were teaching English. It was a great way to integrate with the locals." Before long Matt was teaching English for up to 3 hours on Monday, Wednesday and Saturday mornings. There were also 30minute lessons every evening. Between plumbing, maintenance and teaching, Matt says he virtually worked 6 and half days he'd get some time off on Sundays to explore the shattered city. ABOVE: Luxury cruiser, about five kilometres inland from the Banda Aceh coastline. Most Pfizer Australia colleagues will be aware that our company donated AUD$16 million globally to the Tsunami relief effort and approximately AUD$61 million in healthcare products. But less recognised are the pro bono efforts of some of our colleagues, like trained plumber and Pfizer Australia sales representative Matt Lloyd in Banda Aceh. Like many Australians, Matt was moved by the destruction caused by the Tsunami. His relatives had lived through Cyclone Tracey, which devastated Darwin on Christmas Eve, 1974. But to Matt, the horrors of Boxing Day 2004 appeared almost surreal. "I heard a radio interview with Father Chris Riley from `Youth Off the Streets', " Matt recalls. Youth Off the Streets supports disadvantaged young Australians. "Fr Chris was discussing his organisation's involvement in the Tsunami relief effort. Youth Off the Streets was supporting an orphanage established by the Muslim charitable organisation Muhammadiyah, " Matt said. By chance, Matt had collaborated with Fr Chris and his organisation a number of years ago, primarily providing plumbing support. But with a background in health and fitness he holds a Diploma in Exercise Science ; Matt also spent time talking with teenagers about the dangers of obesity. Soon after the Tsunami struck, Matt contacted Louise Burton, Pfizer Australia's Manager, Corporate Social Responsibility, to discuss his unique skills, background and how he might contribute. Matt was given the green light from his sales team to contact Youth Off the Streets and within weeks found himself in Banda Aceh. "I'm the only trained plumber employed by Pfizer Australia and the company quickly recognised I could support the relief effort, " Matt said. "Clean water was essential to maintaining sanitation in Banda Aceh." With that said, Matt was prepared to take holiday leave to go to Banda Aceh. "But my manager, Trevor Howells, organised special leave, " Matt said. "I received my regular salary and didn't need to take holiday leave." Matt says this was a tremendous gesture from Pfizer Australia. "I'd ABOVE: Pfizer Australia's Matt Lloyd with pupils at a Banda Aceh school. The orphanage was located about 10 to 15 kilometres from the worst affected areas. But despite the distance, Matt says everything was wiped out except for the local Mosque and a few houses. "I remember after Cyclone Tracey blew through Darwin, housing frames were still visible, " Matt said. "In Banda Aceh, nothing was left, only piles of rubbish. It was like a rubbish tip that went for 4 or 5 kilometres." There were other images Matt will never forget, like the massive luxury cruiser wedged between two buildings, many kilometres from the coast. "I'd see things but still couldn't believe what I was seeing, " Matt said. While Matt Lloyd says he achieved his plumbing goals, it was his work with the orphaned children that proved most satisfying. "On the last day, the kids gathered to see us off and there were tears all round, " Matt recalls. Of his work with the children, Matt says: "No matter how hard some days were, I always tried to be as upbeat as possible. I was someone the kids could laugh with or at.

Eluted at a flow rate of 1 ml min-1 with a solution of 340 mg l-1 sodium dodecylsulfate and 680 mg l-1 KH2PO43H2O in a 30: 70 v v ; acetonitrile water mixture of pH 3.0 adjusted with diluted phosphoric acid ; . Kinetically controlled enzymatic synthesis of -lactam antibiotics at pH 7.0 was carried out by mixing enzyme with solutions of activated acyl donor [either phenylacetamide PAA ; , D-phenylglycine amide PGA ; , p-hydroxyphenylacetamide HPAA ; , or D-p-hydroxyphenylglycine amide HPGA ; ] and an appropriate -lactam compound 6-APA or 7-ADCA ; . The initial concentration of acyl donor was 15 mM in all experiments, whereas the concentration of -lactam acyl acceptor varied between 1 and 190 mM. All reactants were monitored in time by HPLC analysis and initial rates of formation of the antibiotic vPs ; and the hydrolyzed acyl donor vPh ; were determined. Peak areas were related to the concentration of the respective compounds by calibration curves that were established with solutions of the pure compounds. Only for p-hydroxypenicillin G, no authentic response factor mM-1 ; could be determined due to the lack of a commercially available reference compound. However, as response factors of compounds varying only by the presence of a p-hydroxy group PGA and HPGA, ampicillin and amoxicillin, or crphalexin and cefadroxil, respectively ; , were found to be very similar 10 % difference ; , we approximated the response given by p-hydroxypenicillin G with the one obtained for penicillin G. 2.6. Chemicals Ampicillin, cefadroxil, and ceohalexin were purchased from Sigma; HPAA was from Acros Organics. Penicillin G, amoxicillin, 7-ADCA, 6-APA, PGA, and HPGA were provided by DSM Life Sciences Delft, The Netherlands ; . NIPAB and NIPGB were synthesized by reacting phenylacetic acid chloride and D-phenylglycine chloride, respectively, with 5-amino-2-nitro-benzoic acid in a water acetone mixture. PAA was prepared by adding phenylacetylchloride dropwise to a concentrated ammonia solution, resulting in the formation of a white precipitate, which was filtered off and dried to constant weight. D-Phenylacetyl-L-leucine was obtained through standard organic chemical peptide coupling chemistry. During the synthesis, consecutive washing of the di-protected dipeptide with acid and base ensured that absolutely no free leucine remained in the sample and cyclobenzaprine and cephalexin.

MRSA CA-MRSA ; strains is an emerging problem. CAMRSA strains contain various virulence factors for example, the Panton-Valentine leukocidin gene ; that confer a propensity to form abscesses, particularly in the skin. Systemic infection with a fulminant onset and subsequent death after seemingly minor skin abrasion and infection has been reported in otherwise healthy young people. The definition of a CAMRSA strain differs between publications, but the essential characteristics are a resistance to standard anti-staphylococcal antibiotics such as cephalexin and flucloxacillin ; and a continued susceptibility to clindamycin and or erythromycin. Rates of infections due to CA-MRSA are increasing and should be considered in certain risk groups table 3 ; , including patients who participate in contact sports eg, rugby, wrestling ; . Sharingtowels or spas and failing to use sterile techniques in medical aid contribute to this risk. All patients who fall into high-risk groups and present with skin abscesses should have a swab taken of a preferably ; purulent skin lesion. In patients with confirmed CA-MRSA or those for whom conventional therapy fails, clindamycin is usually the drug of choice, but susceptibility results are important. For therapeutic purposes, strains that are resistant to erythromycin but susceptible to clindamycin on laboratory tests should probably be regarded as clindamycin resistant, as 90% of such strains usually possess inducible resistance to clindamycin not detected by standard susceptibility tests. In general practice, other treatment options, if supported by susceptibility results, include co-trimoxazole or minocycline. Some antibiotics, such as rifampicin in combination with fusidic acid ; or the new antibiotic linezolid Zyvox ; , are only easily available in hospital settings, so specialist assistance may be required.
